Why the format matters

Applications fail on paperwork far more often than on eligibility.

Every funding program wants its quotes laid out a particular way, with particular product codes, particular supporting documentation and particular signatures. Get any of it wrong and the file comes back, weeks later, and the client waits.

We prepare quotes in the format each program expects, because we submit to all of them constantly and we know what gets returned. For an authorizer that means one less thing to check, and for the client it means the application moves the first time.

Quotes

Formatted per program

ADP, ODSP, March of Dimes, WSIB and VA each want something slightly different. We produce what each one expects.

Codes

Correct product codes

Device codes and categories matter to the reviewer. We use the ones the program recognises, not our own catalogue numbers.

Turnaround

Quotes back quickly

A quote that arrives after a clinic day is a quote that costs a week. We prioritise professional requests.

Continuity

The same consultant

You deal with one person who knows your caseload, rather than whoever answers the phone that day.

What we hold

Four kinds of document.

Tell us which you need and for which product category, and we will send it. Most requests turn around the same day.

Order forms By category

Product-specific order forms for wheelchairs, seating, beds, lifts, stairlifts and bathroom equipment, with the fields the manufacturer requires.

Sent by email on request.
Assessment forms For authorizers

Measurement and assessment sheets for seating, mobility and home access, laid out for the information funding programs ask for.

Useful before a home visit.
Funding documentation Program specific

Quotes, product specifications and supporting letters prepared in the format each program requires.

We submit these ourselves too.
Product literature Manufacturer

Brochures, specification sheets and dimension drawings from Handicare, Savaria, Access BDD, Quantum, Pride and the rest.

Ask for a specific model.

Working with us

What professionals tell us actually helps.

Occupational therapists and case managers are not short of suppliers. What they tell us they are short of is accurate quotes that arrive when promised, equipment that matches what was specified rather than something similar, and a consultant who remembers the last case.

We are not going to claim to be unique in this. We are saying it is what we try to do, and that if we fail at it you should tell us, because it is the entire basis of a professional relationship.

Quotes in the format each funding program expects
Trial equipment available for seating and mobility assessments
One consultant per caseload rather than a general line
Delivery and installation coordinated around your schedule

Other things professionals ask for

Beyond paperwork, the practical support that comes up on most cases.

Pressure relief cushion available for trial before specification

Trial equipment

Chairs, cushions and seating for trial before specification. Far more useful than a catalogue for a complex seating case.

Joint home visits

A consultant attending with you, so measurements, equipment options and funding are settled in one visit rather than three.

Power wheelchair with configurable rehab seating for custom seating work

Custom seating

Complex seating and positioning built to a prescription, with the measurements taken jointly and confirmed before manufacture.

Hi-low electric hospital bed supplied to long term care facilities

Facility supply

Bulk and standardised supply for long term care and retirement homes, with consistent specifications across a floor or a building.

Paying for it

We submit to all of these, constantly.

ADP, ODSP, WSIB, March of Dimes Canada, Veterans Affairs Canada, the Interim Federal Health Program and BlueCross. Being an authorized vendor for all seven means we know each one’s quote format, documentation requirements and current turnaround.

If you are authorizing a device and want the quote formatted a particular way, or need supporting specification sheets attached, say so and we will produce it that way rather than sending a generic PDF.

Questions from professionals

If yours is not here, call us on 905 451-7743. A person answers during business hours.

How do I request an order or assessment form?

Email the order desk at [email protected] or call 905 451-7743 and tell us the product category. Most requests go out the same day. If you work with us regularly we will set you up with a consultant who holds your caseload.

Can you provide quotes in ADP format?

Yes, and in the formats ODSP, March of Dimes, WSIB and Veterans Affairs expect as well. We submit to all of them regularly, so we produce what each reviewer is looking for rather than a generic quotation.

Do you provide trial equipment for seating assessments?

Yes, subject to availability. Cushions, backrests, chairs and seating components for trial before specification. For complex seating cases this is usually more useful than any amount of catalogue reading.

Will a consultant attend a home visit with me?

Yes, and it is usually the most efficient way to work. Measurements, equipment options and funding routes get settled in one visit rather than across three. Give us as much notice as you can and we will match the schedule.

Do you supply long term care facilities?

Yes. Bulk supply, standardised specifications across a floor or building, and consistent product so staff are not learning a different handset in every room. You deal with the same consultant each time.

How quickly do you turn quotes around?

We prioritise professional requests because we know a quote arriving after your clinic day costs a week. Straightforward quotes usually go out same or next day; complex seating and custom builds take longer and we will tell you how long.

Can you supply manufacturer specification sheets?

Yes. We hold literature, specification sheets and dimension drawings from Handicare, Savaria, Access BDD, Quantum, Pride, Sunrise, Motion Composites and others. Ask for a specific model and we will send what the manufacturer publishes.

Who do I contact about an existing order?

Your consultant directly, or the order desk on 905 451-7743. If you tell us the client name and roughly when the order went in, we can usually tell you where it is without a callback.

Request a document

Tell us which form or document you need and we will email it, usually the same day.
