Description

The Power Plus Extreme is a Category 5 manual dynamic tilt wheelchair built in Canada by Power Plus Mobility. Westin Healthcare configures it in Brampton after an ADP-registered occupational therapist or physiotherapist writes the prescription. The Ontario Assistive Devices Program lists this frame as WM0001041. It is a gas-strut tilt, not a power chair and not a battery product.

Power Plus prints a 0 to 25 degree tilt that keeps the front seat-to-floor height steady, a depth-adjustable frame, dual cylinders, a seat-to-floor range from 12.25 to 18.25 inches, a chair weight of about 60 lbs, and a capacity that starts at 250 lbs and can be built up to 600 lbs. This is the Extreme, not the Super Tilt Plus (WM0001040).

Who this chair is for

This chair is for a person who needs a manual dynamic tilt frame because seating parts alone do not keep them upright, and whose therapist has placed them in ADP Category 5. Families usually call us when they want a lower 25 degree tilt that holds the front seat height, a recessed frame for foot propulsion, and a chair that can be pushed or self-propelled once the wheels are set.

If the person needs the higher-tilt Super Tilt Plus, use the Power Plus STP. If they need a 45 degree Maple Leaf frame, see the Maple Leaf SuperTilt. A transport chair with small rear wheels is a different product.

Design and build

The Extreme is a light-weight, depth-adjustable tilt frame with an ABS seat pan. Two gas cylinders move the seat from 0 to 25 degrees. Power Plus writes that the front seat-to-floor height stays the same through that tilt. A two-way locking strut and lockout collars hold the angle. The lower frame is recessed for foot propulsion. Axle plates are recessed.

Arms are T-style and come off. The stroller handle adjusts from 0 to 180 degrees. Front hangers swing in and out. Caster journals adjust. Back angle is 0, 5, 10, or 15 degrees. Power Plus prints an overall width of 29 inches on an 18 inch seat. The current factory is in Guelph, Ontario. Older forms still show the Brampton Wilkinson Road plant. Westin also sells their Relax Multi-Axis Headrest (MAHF) when the therapist writes one on the order.

Owner-manual warranty for the original purchaser: two years on workmanship and materials, five years on the frame and cross-braces. Tires, tubes, arm pads, and upholstery are wear items and sit outside that term. Accident, misuse, unauthorized repair, and non-factory extras void the term. Labour and freight are not covered.

Seat width, seat depth, colour, occupant weight, hangers, and wheels are written on the Power Plus order with the therapist. Westin builds the Extreme to that form. This is not a same-day stock chair.

ADP funding for a Category 5 wheelchair in Ontario

This chair is an ADP-eligible category device. Ontario's Assistive Devices Program (ADP) pays 75% of the approved price for eligible clients, and clients receiving Ontario Works, ODSP, or ACSD support can be covered for the full cost. The route starts with an assessment by an ADP-registered occupational therapist or physiotherapist, who confirms the need and the right category before anything is ordered: a chair bought before approval is not reimbursed. What does Category 5 mean on this wheelchair?

Category 5 is Ontario ADP's adult manual dynamic tilt class. It is for a person who needs the seat to tilt because seating parts alone do not keep them upright. The Extreme is listed on that table as WM0001041.

Is the Extreme a power wheelchair?

No. The Extreme uses two gas cylinders and a manual release. There is no drive base and no battery on the standard chair. Ignore any leftover line that talks about powered tilt or a long-lasting battery.

How is the Extreme different from the Super Tilt Plus?

The Extreme is a 0 to 25 degree dynamic tilt that holds front seat-to-floor height. The Super Tilt Plus is ADP WM0001040 and uses a higher tilt (Power Plus prints up to 55 degrees on the STP spec table). They are two codes, not two names for one chair.

Who qualifies for ADP on a Category 5 chair?

You must be an Ontario resident with a valid health card and a disability that needs a mobility aid for six months or longer. An ADP-registered occupational therapist or physiotherapist has to assess you before the application. Westin cannot promise that a named person will be approved.

How does the 75 / 25 split work?

For an eligible client, ADP pays 75 percent of the approved price and the person pays 25 percent. Ontario Works, ODSP, or ACSD can open 100 percent of the approved price. Extra items that ADP does not list are the person's cost. Do not buy the chair before approval if you want that funding.

What is the assessment process?

You see an ADP-registered OT or PT. They confirm the need and the category, then write the Extreme configuration with Westin. We help complete the application and submit it. Ontario says it aims to review a mobility application within eight weeks.

Can I try an Extreme before it is built?

Come into a Brampton or Toronto store and we will show you a tilt chair on the floor if one is there. A floor chair is rarely the exact seat and colour on your order. The therapist may also arrange a trial through the clinic.

What seat width do I need?

The therapist measures seated hip width. Power Plus prints 14 to 36 inches of width and 16 to 24 inches of depth on the Extreme brochure. Those numbers are set on the order form, not guessed at checkout.

What is the weight capacity?

The 30 April 2026 order form starts at 250 lbs and has heavy-duty steps up to 600 lbs. Stay at or under the rating written on your chair. Do not mix this with the STP page's 250 / 400 / 600 story unless your order is an STP.

What is the warranty?

Two years on workmanship and materials, five years on the frame and cross-braces for the original purchaser. Wear items are excluded. Westin Healthcare processes the claim.

What happens if ADP declines the application?

Ontario will send a letter with the reason and the appeal path. Do not take delivery against an ADP promise until that letter is in. We can talk through a private-pay order or another frame. Call (905) 451-7743 or use the funding page.
